Reader Question – Post-operative Constipation?

What do you do for post-operative constipation?
After any surgery involving anesthesia and post-operative narcotics, patients are often constipated and at times nauseated. Most cosmetic patients have not had internal abdominal surgery, so dehydration and narcotics are usually the reasons.
I tend to advise taking it easy on the narcotics if possible. Adding Psyllium, a natural fiber laxative, with liberal water intake can relieve things without the need for anything more harsh. If you are under the care of another surgeon, you shouldn’t add anything without letting him or her know.
One of the reasons I am so fond of pain pumps for tummy tuck patients is to reduce their need for narcotics after surgery. This really diminishes constipation.

Jane Fonda – Not Proud of Plastic Surgery

Jane Fonda “caved” in to getting plastic surgery after vowing in 2000 she would not get any more work done, she told Larry King Monday night.
“If I was really brave, I would have not,” she said. “I vowed I wouldn’t–I did, and I don’t feel proud of it.”
Plastic surgery is really not something of which to be proud. Hollywood types do take interesting points of view on it. There are those who seem to overindulge who are highlighted for effect. Others tell the public not to have plastic surgery and then have it themselves.

Kate Gosselin is no stranger to plastic surgery, but I would need to see a fair number of recent images to figure whether or not surgery was involved in her recent makeover. She has admitted to recent hair extensions and dental work.
Take into account that the cost elements quoted in most news stories are wild guesses. According to the links above she paid $8000 for a tummy tuck and $11000 on “skin procedures on her cheeks.” As a woman who hosted six babies in that one pregnancy, she would have needed much more than an $8000 tummy tuck. Maybe she got an extreme discount?

ObamaCare Will Cost You…Goodrich Corporation

Goodrich Corporation (NYSE: GR) announced today that it expects to incur a one-time charge against its first quarter 2010 results of approximately $10 million, or $0.08 per diluted share, due to the recently-passed U.S. health care reform legislation.
The expected charge relates to the elimination of tax deductions available to companies that provide prescription drug coverage to retirees.
The health care reform bill will cost us money. We continue to watch.

Kate Hudson – Small Breast Implants Look Nice

GROWTH AT AGE 31
Actress Kate Hudson, 31, apparently decided she didn’t want to be so flat-chested any more. In recent photos, she shows signs of having gotten small breast implants in a recent operation, reportedly last month.
Ms Hudson has definitely had a little surgical help and it looks nice. Small implants do better than larger ones over time particularly in thin women.

Doctor Sues Allergan Over Botox

The latest suit was filed by Dr. Sharla Helton, 47, an obstetrician and gynecologist in Oklahoma who fell ill and eventually lost her job after getting Botox injections in 2006 to smooth her wrinkles.
She blames Botox for weakness, shortness of breath, double vision, and years of continual pains in her arms, hands, and feet that began after her Botox shots.
It seems like there is a line to sue Allergan, the maker of Botox. This OB/GYN has developed some kind of neuromuscular disease and of course is blaming it on Botox. Allergan has lots of money and these suits will will likely continue.

Hospitalist? What’s a Hospitalist?
On my way to the gym recently they were talking on KABC talk radio about Hospitalists. They left out a few cogent points and I couldn’t get to them before they switched topics.
Hospitalists are doctors who specifically care for hospitalized patients. The problem as I often see it is how they work and more specifically for whom they work.
Generally Hospitalists work for an IPA or HMO. They are hired to facilitate efficient (read “Cheaper”) Care. They get patients in and out of the hospital operating primarily to make it cheaper for their employer.
When you are admitted to the hospital and you are an HMO patient, if that HMO has a hospitalist program, the hospitalist on duty takes care of you. They work in shifts. You regular doctor is often not even informed of your hospital visit.
This is just another ding to the continuity of care that used to be one of the best aspects of a great Health Care system.

Reader Laura on Wrong Breast Implant Lawsuit

An Ohio woman says her plastic surgeon gave her the wrong breast implants. She requested silicone and the Dr’s office only had saline. The patient also claims that she was kept under longer than she should have been while the Dr convinced her fiancee to agree to the change.
I don’t know about this one. The fact that the woman is asking for $300,000 dollars “to have the surgery corrected and for physical and emotional discomfort” leads me to believe that this is a nuisance suit. An implant exchange is not a big operation usually. Did she pay for silicone or saline implants?

Amy Winehouse And Her Painful Breast Implants

TERRIFIED AMY WINEHOUSE has been rushed to hospital with agonizing pains from her boob implants.
The 26-year-old Rehab singer was kept in overnight while doctors carried out urgent tests.
She is now waiting to discover whether she must undergo corrective surgery – or even have the 32D implants removed.

This is an old report. Entertainers (particularly those addicted to narcotics) frequently have dramatic stories with plastic surgery. In twelve years of practice, I have yet to have a patient hospitalized with painful breast implants months after surgery.
She could have the formation of firm capsules which can be painful particularly if she has silicone gel implants and smokes cigarettes. Hospitalization for urgent tests is over the top and entirely uncharacteristic however. Entertainers tend to have more exciting plastic surgery than is the norm.
