Reader Laura On Dental Course on Botox and Fillers

This is a company that is offering training for “Botox® and Dermal Fillers for Every Dental Practice”. They are offering a 1 day course to train Dentist on how to use Botox and fillers. How much training do plastic surgeons receive to do these procedures? How likely will it be for a regular dentist to be able to perform these procedures without mishaps?

I could teach a monkey to inject Botulinum toxin. Plastic surgery residents pick it up very quickly. Fillers are just about as easy as long as reasonable limits are observed. Some users do not know where to stop and/or inject the less safer “semi-permanent” fillers in large quantities. This can lead to scary results.
I was actually approached by a CME company not long ago about teaching such a course. It would be pretty easy. The question of how good the attendees will actually be at the procedures and what their judgment might be is another question entirely. In the right hands with the right judgment, internists inject these items safely every day.

Heidi Montag Surfaces to Bag Plastic Surgery Again

Heidi Montag is a plastic surgery machine…first for having more than the average woman could have in a lifetime before the age of twenty-five and then for whining about it at every opportunity. In dire need of publicity she has surfaced once again like an enemy Uboat so we can all deal with her again. I talked to the OC Register about it:
“Though she had perfectly smooth legs prior to the surgeries, Heidi was horrified when photos surfaced depicting what appeared to be cellulite on her thighs just a few months after the body-contouring procedure. “I can’t believe I did this to myself,” she laments.
My comments:
“Heidi has lost her luster to the entertainment community. I’d wager she would say anything to make the powers that be happy, and bagging on your plastic surgery is popular now.
“With that being said, lipo is not a solution for all. As we have seen in other entertainers (e.g. Tara Reid) lipo can leave patients with irregularities seen in the treated skin. This is not a common occurrence, but happens much more prominently in cases in which it is performed inappropriately.
“There has to be enough fat to treat and the skin over it has to have the appropriate tone or elasticity. These are issues judged properly by trained personnel. Philosophy also has a good deal to do with which patients are offered surgery in a particular practice.
“Heidi was quite thin before surgery and is likely to have had surgery in areas that were not good targets … too little fat to treat. I’d bet that is the source of her troubles if they exist to any significant extent, anyway.”

Locum Tenens Plastic Surgery in Southern California?

I am occasionally contacted by agencies looking for Locum Tenens plastic surgery coverage. As I am selective, I have not to date taken such an assignment, but want to stay in the loop vis a vis opportunities. If one looked good enough, I might try it.
Locum tenens jobs have traditionally been part time assignments in a particular specialty. Frequently they amount to a doctor on vacation looking to cover the care of his or her patients while he or she is absent. They are relatively rare in plastic surgery and are usually for non-cosmetic business for a hospital. They very rarely encompass cosmetic surgery. Patients do not usually want an alternate surgeon performing their cosmetic surgery. They are paying for the experience of the doctor with his or her name on the door.
Recently I received an email from such an agency looking for Locum Tenens plastic surgery specifically for cosmetic surgery in San Diego. Seeing as it was so unusual I inquired as to what they were paying. It was $1200 for a day of surgery. In a plastic surgery practice, they will get no one of any reasonable skill for that. It is below what it costs a quality doctor to operate. They apparently want a few days a month.
This is potentially scary. It might represent a chop shop in cosmetic work trying to get started in San Diego. They were specific about wanting a surgeon who could do breast implants, liposuction and facelift surgery so it is not just for small stuff like fillers and botulinum toxin.
From what the service would tell me, this is a group of surgeons looking to pay cheap rates for an operating cosmetic surgeon. It is probably a business enterprise by a group on non-plastic surgeons looking to make money on cosmetic work via another doctor.
Buyer beware.

“A woman who’s going by the name Vanity Wonder has written a book about her experiences as a black market patient getting illegal injections to becoming an assistant to one of the practitioners. Looking at pictures of this woman it looks like she has permanently disfigured her body. What future health issues does this woman have to look forward to especially since she admits to not even knowing what all was injected inside of her? Is there anything that she can do to remove the gel that was injected into her hips and buttocks? I have to wonder what she is going to do when the fad of larger than life backsides goes away.”
In a raw and honest account, Vanity Wonder, 30, a mother-of-two from America’s Midwest, has revealed how she became addicted to silicone shots to increase the size of her behind. Shot Girls sees her detail her own experiences of seeking out and having the injections, and how she became an assistant to a black market practitioner, who treated women as varied as a 17-year-old brought in by her proud mother and even a senior in her seventies with bifocals and false teeth.

We have discussed before the dangers of large volume injections to augment the size of the booty and thighs. People have died engaging in these treatments if you could even call them that.
The practice of injecting these varied and unknown materials into one’s body is foolish because quite frankly we do not know all that might happen later. She can have them surgically removed but it will more likely than not leave her looking quite deformed. The materials might become infected, scarred or be ejected by the body. This can happen with silicone injections as well as any other incompatible material. Embolization (traveling of the material through the veins) is possible although this tends to happen earlier as opposed to later. Embolization as we have seen can be fatal.
But society has changed. Instead of calling these people for what they are (because that would be “hating,”) we give them a pass by branding them “victims” and “addicted.” This woman will even profit from a book deal. Society used to brand people engaged in foolish behavior and by doing so others were discouraged from following in their tracks. When people profit from foolishness, the potential for someone else to go down that road just increases all the more.

Reader Question: Octomom Porn and Labiaplasty

“Octomom’s porn came out and she’s gotten an EXTREME labiaplasty that people are noticing and commenting on twitter and TMZ. If you can stomach it, would you be able to comment on that and her obvious body lift scars?”
We have discussed Octomom and her plastic surgery exploits here before. She must think she is a celebrity because she has been in (at least) public denial about obvious body lift scars. How a woman on public assistance with over eight children affords expensive plastic surgery is a difficult concept.
On the question of labiaplasty, I have not seen images and am not sure I would want to see images. With the Octomom porn out, certainly many have seen her privates. It would not surprise me if Ms Suleman had had labiaplasty surgery and went extreme on the removal. We have discussed the wide variability in the practice of labiaplasty here before. Some women want their lips completely removed which is not my practice. I favor reduction not complete amputation. Then again I am not Octomom’s surgeon.

Alicia Douvall Complains About Cheek Implants

Now Alicia, who has spent more than £1million on more than 100 cosmetic procedures — and almost died several times from surgical complications — admits she was emailing cosmetic surgeons abroad from her maternity ward.
However, this time it was for corrective surgery to fix dodgy facial implants she had done before she realized she was pregnant which had disfigured her face and left her unable to smile.

We have discussed Alicia Douvall here before. When someone says she nearly died from cosmetic surgery complications, it gets me thinking:
(1) She seeks extremes in surgical talent, goals or both.
(2) She is at least a bit dramatic.
As I have been in practice for about 15 years and have never seen a patient in a life or death position due to properly-performed cosmetic work, I can’t help but wondering if these stories are just the machinations of an overactive mind.
On the issue of dodgy facial implants, she probably went too large. Whether she demanded the implant size involved we will probably never know. She will likely blame her surgeon.

Reader Laura on Tanning Bed Safety and Salon Fines

“The tanning salon where the now infamous tanning mom took her daughter has been hit with over $5,000 in fines. Some of the claims are that they overexposed their clients. Are there any tanning booths or exposure to these booths that can be considered safe? How can you be sure that you are going to a safe tanning salon or is there even such a thing?”
The salon where tanning-mad mother Patricia Krentcil has been accused of taking her five-year-old daughter has been given a hefty fine for breaching a number of rules. City Tropics Tanning Salon in Nutley, New Jersey, has been hit with a $5,200 fine by the state health department following a check by officials on May 14. The string of violations at the salon where Ms Krentcil received her distinctive dark orange skin coloring included overexposing several customers in tanning booths.

I have railed about tanning bed safety before. Tanning salons and their equipment have been linked to increases in skin cancer and aging changes to the skin, so as far as I am concerned there are no safe tanning booths.
It is a matter of philosophy as to whether or not people should be allowed to expose themselves to such risk, but tanning salon owners have not been so honest about risk. The basis upon which the officials fined them would be interesting to know. At what level of exposure does the State call the event illegal? That seems arbitrary to me.

Angels Baseball 061812 – Dr D’s Comments

We watch the Angels in Dr D’s house, so a few (medical) thoughts after last night’s game:
(1) Kendrys Morales / Erick Aybar – Quit the chewing tobacco. Remember Tony Gwinn.
(2) Albert Pujols – After getting hit by that pitch on your hand (right over the tendons) I would stay away from non steroidal drugs like Motrin, Advil and Naproxen for the next few days or it will bruise and swell which might become a problem with your batting / fielding.

Breast Implant Test – Under a Car

The home tests that people put breast implants under are something else. Here someone actually ran a breast implant under a car. While this is not exactly recommended activity, it can provide a little humor to pass the time.
