In looking into larger breast implant sizes and the way you want to look, there are a few things that come into play:
Your current breast tissue: How much is there? Some women want a very high round fake look that their current breast tissue works against. Normal breast tissue tends to look natural. It hangs a bit. Some women like this “natural look.” Others don’t.
Your willingness to risk things going wrong: The larger you go the more the chance that implants will droop, ripple, “firm up” or hurt. These chances get larger as the implant size gets larger.
There are ways reduce some of this risk and they generally involve doing a few operations to slowly increase the breast size and stopping at some point.
I look at these cases relative to the risk and how reasonable the patient seems. Many ladies just want moderately large and this is quite do-able.

I am not a doctor. As a breast augmentation enthusiast, I have learned that women with very large breast implants typically have multiple surgeries with a 60% size increase each surgery.
Over-the-muscle placement gives the roundest, most fake-looking result, by letting the shape of the implants show through the overlying tissue. In contrast, under-the-muscle placement will flatten and obscured the implants.
Women with very little natural tissue can minimize rippling by using overfilled saline implants. This also contributes positively to projection and round shape.
By getting breast implants, a woman has already taken a risk. Very large implants carry some additional risk, but it can be worth it for a woman to be satisfied with her results.

I think it comes down to what the woman wants and what she is willing to do to get it. Things are a little more complicated than that which you write. In this market, very large implants come with more liability for both the surgeon and the patient. It is all relative.
