The real bottom line (no pun intended) is that women need to be careful what they seek and who they allow to operate “down there.” When the surgery is carefully limited to the outer structures, this kind of stuff doesn’t happen. If you just seek to reduce the outer lips and your experienced surgeon uses the right technique, you are pretty safe. Those seeking vaginal tightening can fall prey to the wrong kinds of surgeons.
Just as important is the fact that even nightmares can be improved when the patient finds the right surgical talent.

The lighting has changed but the anatomy is the same. Certainly not a fake.

That could easily be the differing light levels under which the images were shot. I think they are very likely real.
