Reader Question:
I am looking into getting breast implants but want to keep redo surgeries to a minimum. Some of my friends have had 2-3 boob jobs in ten years, so I am not going to their doctor. I saw a few of your patients online saying that the implants you put in they kept for ten years. I want to get a worry-free boob job?

There are several things you can do to reduce your risk for the need/desire for re-operation after breast implant surgery:
(1) Have surgery by a good qualified surgeon. In my book, that means a good ABPS board-certified plastic surgeon.
(2) Request guidance from your surgeon regarding the volume to which you can go safely. The leading cause of poor outcomes with implants that I see is the placement of implants too large for a woman’s available soft tissue coverage. Good surgeons know this although many fear “losing the case” if they share their concerns.
(3) Choose saline-filled implants and have them placed beneath your pectoral muscles. Silicone gel is another option but I feel the breast hardening rate (capsular contracture} is higher with them. Capsular contracture is also higher in some other cases that you may want to review before surgery.
(4) If you have any degree of breast sag, talk about breast lift surgery as an option before you have your operation.

Porn Stars and Their Boob Jobs

I wrote an article for “Exotic Dancer” magazine a few years ago in which I discussed the problem with many adult entertainer / porn star breast augmentation outcomes. The funny thing (when looked from outside the practice) is that women in general come to plastic surgeons wanting pretty large breasts. At times, the requests are destined to look unnatural or perhaps even freakish.
Personally I turn away the freakish ones. I don’t do this very often. 
When women “go really large” and have little of their own tissues available to cover the implants they not only end up looking unnatural, but also have a higher chance of hardening (capsular contracture), rippling (showing signs of the underlying implant – visible through the skin) as well as other complications. These complications usually amount to more surgery.
This is all relative; relative to the amount of tissue you have over your chest to begin with (skin, breast, and muscle) compared to the dimensions of the implant you’d like placed. Many women can get the size they want and not end up with problems. If you are very thin though, maybe those DD’s should be off the menu.
Really pretty women can end up with funky-looking breast implants. These kinds of outcomes can be avoided many times by simply choosing smaller implants (or avoiding high profile implants.) Choosing qualified surgical talent and following the guidelines such surgeons offer can be helpful as well.
When patients follow my advice, their implants tend to last ten years or more without additional surgery. Ask how many of your friends have seen that kind of longevity with their implants. I call this low maintenance breast augmentation.

Once in a while prospective patients inquire asking whether or not I can do a “porn star style boob job.” We usually answer the question with a few questions:
How Large Are You Looking to Go?
&
How Do You Want To Look?
In looking into larger breast implant sizes and the way you want to look, there are a few things that come into play:
Your current breast tissue: How much is there? Some women want a very high round fake look that their current breast tissue works against. Normal breast tissue tends to look natural. It hangs a bit. Some women like this “natural look.” Others don’t.
Your willingness to risk things going wrong: The larger you go the more the chance that implants will droop, ripple, “firm up” or hurt. These chances get larger as the implant size gets larger.
There are ways reduce some of this risk and they generally involve doing a few operations to slowly increase the breast size and stopping at some point.
I look at these cases relative to the risk and how reasonable the patient seems. Many ladies just want moderately large and this is quite do-able.

A young bank clerk who stole £46,000 from her employer to help achieve her dream of becoming a model told police she had earned the money from working as an escort. Mother-of-one and law graduate Rachael Martin, 24, ‘spent money like water’ after getting a job with Barclays in Liskeard, Cornwall, where she was responsible for dealing with cheques.
The stolen cash paid for a breast enlargement, thought to be worth £4,000, dental work worth £1,700, and liposuction, as well as nights out, drink and drugs. She has now been jailed after stealing £46,000 in just two months from her employers.

This 24 year old law graduate stole from a bank to fund plastic surgery and nights on the town. Interestingly she by report told those who asked about the source of the money for her indulgences that she worked as a prostitute. The judge apparently had pity on her when he sentenced her. I am sure that will make the bank that is out the money quite happy.

Reality stars run at opposites with conventional Hollywood types in many ways. One of them is admitting (well OK bragging about) plastic surgery. And that is good for us here!
There are a few worthwhile points to this dish session. Firstly breast implants are not forever. Ang reports on three boob jobs starting in 1985. That figures correctly as about one on average every ten years.
Going to J cup size with breast implants is not possible or practical for most women, but it obviously works for her.
One other point: She talks about her age “kicking in.” It happens and fat can start piling up if diet and exercise are not implemented. Also sponges were not standard breast implants in 1985. I wonder where she had her original surgery.

Instant Boob Job

It is undeniable that they did make me feel more feminine and a whole lot sexier. But they were fantasy breasts, and accepting that made them easier to take off for good. Because they were made of silicone, they were pert and lifted, and perfect. Most breasts ? like so much of life ? are imperfect. They sag and droop. I will never have large breasts again, though, and that makes me sad. They are sitting on my dresser now, a silicone reminder of some fleeting memories of being a size DD.
