Good Plastic Surgery in the OC – A Good Redo Boob Job

Here we have images of a woman I first saw a few years ago. She had had two breast implant operations before and noted after her second (an enlargement with full lift) that her left breast progressively dropped (over about a year). I recommended re-operation to re-set the lift and downsize her implants. She agreed to the lift but not the implant size decrease. Smaller implants do not weigh as much so the rate of recurrent sag should have been less. As these were silicone gel implants this is particularly important. They weigh more than saline-filled implants.

The After image was taken at 1 month. She moved Out of State, but I predict that the sag probably recurred at least to some extent.

The Lesson:

(1) Large implants are hard to keep up. Plan on smaller implants or more surgery later if you select larger implants. Large silicone implants also harden (capsular contracture) more frequently/severely than smaller implants or saline implants. This also contributes to a higher redo rate.

Breast implant surgery does not always go as planned. The reasons vary from patients seeking poor surgical talent at their local $3000 chop shop, smoking too many cigarettes or just plain having bad luck.
Much of this can be fixed or at least improved with additional surgery.
This young lady found me after having had four operations with her prior surgeon at which point he uttered those unforgettable words: “This is as good as it is going to get.”

She had poor scarring partially due to her genetics, but the poor shape and firmness was correctable. She found her way to my practice after a friend with whom I had worked in the past referred her to me. We performed a modified breast lift and implant exchange with capsule removal in a local Orange County surgical center and she achieved a nice improvement.

Fixing Bad Plastic Surgery:

** Get into the best hands you can afford and check into your surgeon’s qualifications before you let him operate upon you. Understand that redos are harder and less predictable than first time surgery.

Years ago I did some work for a great looking 40 year old mom who worked out incessantly and had a great athletic body. Despite the working out though, her tummy had some changes from her pregnancy years before and her breasts were a bit small and droopy for her tastes. After a few years and two operations (Breast lift/implants and a tummy tuck,) she sent me this bikini image with a thank-you note.

Plastic surgery can work out well when done properly in the right patient. These are good tummy tuck results, but in reality all plastic surgery results vary. Only few patients get this kind of “knock it out of the park” result. They tend to be the people who are already in great shape. There is no overemphasizing doing your part.

Diet and exercise make you a better candidate for surgery anyway. :)

Reader Question: Ugly Breast Lift and a Mass – How to Avoid / Fix?

I have been considering fixing my saggy sad boobies (two children, breastfeeding – blah. blah. blah.) Then I find this image online of a woman who had a lift with ugly scars and a new lump in her breast. This scares me. How do I get pretty boobies without the scars and lumps this lady got?

Plastic surgery comes with risk. Although the scarring you see here is not typical, it can happen and require surgery to fix. Of course poor scarring occurs more in certain practices than others and in patients who are less healthy and/or smoke more than in others. Compared to what we see in this case, the Katie Price breast lift scar drama kinda pales in comparison. All things are relative when we look closely enough anyway.

This lady you mention has two issues… scars and lumps. They may or may not be related.

(1) Scars – She shows her right breast in your image with a thick irregular scar in the healed incision at the inflamammary (under breast) fold. This looks amenable to scar revision. Her vertical scar between her areola and her lower breast scar is a bit wide as well. I would probably recommend scar revision surgery once these scars had matured and softened. Widened scars are particularly amenable to well performed scar revision more than some of the other treatment options.

(2) Lumps – Breast lift and reduction can result in scarring under the skin and in the breast that feels lumpy. These need to be carefully evaluated to make sure they are really just scar and not cancerous lumps. Fat necrosis after a breast reduction in particular can manifest in lumpy scars that become smaller over the first year from surgery. They can be removed if need be to prove that they only represent scarring and not something more worrisome.

How do you avoid these?

You can limit the problem by picking the right doctor, not smoking and having the right genetics.

Reader Question: Didn’t Get my Breast Lift and Now Looking at Breast Implant Operation #3

I am very confused on how to handle my situation and a friend suggested contacting you. I had a breast augmentation in 2009. My implants never dropped into the pockets. The surgeon informed me that this was a complication some women had. I moved out of State and a year later I decided to go have the issue fixed and get a lift at the same time. When I went for my follow up after surgery I asked about the lift because my breasts did not seem to be lifted. He told me that my nipples were facing upwards now and that was an indication of a lift. Last week I consulted with a third surgeon and he informed me that the surgeon took my implants from under the muscle ( which I did not consent to) and placed them over the muscle without a lift. What would you suggest I do? I paid for a procedure that was not performed and he did things that I did not consent to.

In years gone by when I had more time to spend answering questions in plastic surgery forums online, I would occasionally get one like this. Patients often do not realize that finding a good plastic surgeon and sticking with him serves a number of positives:

(1) Good plastic surgeons communicate well with their patients.

(2) Good plastic surgeons clearly state before surgery what is to be done and if there is conditional work, it is clearly explained. Sometimes it is not clear on the front end as to whether or not a breast lift will be required. More commonly the exact type of breast lift needed can be in question.

(3) While good plastic surgeons are not cheap, their involvement leads to a much lesser chance that this kind of mess can occur. Three breast implant operations in two years is a bit out of hand. I haven’t had a problem case of this magnitude in over ten years at least not one I have operated the first time.

Looking at the questions here, permission can be conditional. If the words “possible breast lift” appear on the consent, then it might be unfair to cry “foul!” For issues involving the law, I suggest patients at least have a discussion with an attorney. I am a surgeon.

Complicated breast implant cases in my experience are more common when the patient has been elsewhere first. Revisions can be hard anyway. It will take a record review (of any records available) and an exam at minimum to figure the best course of action here.

Star Jones Bled during her Breast Lift

Star Jones reportedly had a severe bleeding episode during breast lift surgery. This is extremely uncommon (I have never seen a case such as the one described) and published details of the event are sparse.

Breast Lift Surgery – The Truth

Breast Lift surgery (Mastopexy) is obviously surgery to lift the breast. There are variations of this operation – quite a few variations. Breast lift operations are usually discussed relative to the “full breast lift” which has also been called the anchor lift. This operation leaves scars around the areola, under the breast and vertically between the two. The shape of the scar configuration resembles an anchor therefore the name. The potential for scars is one of the major concerns potential patients have with the surgery. This version of the operation also has the greatest potential to change the shape of the breast.

Reduced scar breast lifts came into creation to limit the potential for scarring. The important compromise however is that these modified breast lifts “lift” less. Reduced scar lifts can involve any portion of the full lift scar pattern. The modified lift based with an incision above the areola only is called a “Crescent lift.” It provides only a very small amount of actual lift. A “Benelli Lift” uses a suture to pull the breast skin in toward the nipple. It leaves a scar around the areola. Sometimes there can be wrinkling in this position which can fade with time.

Breast lifts can involve implants, but breast implant placement by itself lifts very little. Really large implants result in recurrent descent over time. Gravity usually wins. The addition of breast implants to lift surgery does allow the addition of fullness in the upper breast area or upper pole fullness. This is frequently a crowd pleaser.

The key to breast lift surgery is to decide what is important to you and what you are willing to do to get it to happen. The basic bottom line on reduced scar breast lifts is that they have lesser capacities to change the shape of the breasts. That is the trade off for fewer incisions and potential scarring.

Seek consultation with a quality board-certified plastic surgeon to explore your options further. Good plastic surgeons can help limit scarring and guide you in choosing your procedure.

Exercise Instead of Breast Reduction/Lift?

Women sometimes are sold on the idea that exercises will lift their breasts so breast reduction or breast lift surgery can be avoided.

Exercise can lead to weight loss and weight loss can hep reduce breast fullness. This will not lift the breasts however. Sometimes weight loss will make the breasts sag more, but it will not lift them. Exercise does not tighten the skin. If your trainer is telling you exercise will lift your breasts, find another trainer.

I usually recommend that my patients get their weight into a range that they can maintain, then consider whether or not surgery might be a good thing for them.

Katie Price’s Nipple Slip and Reducing Breast Lift Scars

Jordan had just had surgery to reduce her implant size and lift her breasts. Close up images of a nipple and areola after breast lift surgery take time to improve. It is called “healing.”

What do you all expect? Weeks after implant exchange and lift surgery of course things will look funky. This is one of the reasons to think twice before having monster-sized breast implants. They stretch out your skin and if you decide to later remove or downsize them, a breast lift will be needed to “take in all the slack.” Everyone needs to chill.

As an aside, I like the use of fewer sutures over the skin in breast lift surgery to reduce the cross-hatching you see in Jordan’s scar here. As time passes that will get lighter, but will not go away entirely. One of my full breast lift/implant patients is linked below so you can see what I mean. ;)

OctoMom In Bikini Denying Plastic Surgery

OctoMom has once again surfaced like the media submarine that she has been now with a bikini body. Images show hints of photoshop and her body has absolutely (my opinion of course) seen a knife or two to get this way:

My money would be on lower body lift plus breast implants and a lift minimum.
