Dr D Shows You A Rock Hard Silicone Gel Breast Implant Capsule

A short video shows a 27 year old ruptured silicone gel implant and the calcified scar capsule the patient’s body made to try to contain it. It helps explain why some silicone breast implant patients develop rock hard breasts and what can be done to fix the situation.

It also helps explain why most often I prefer saline-filled breast implants that do not tend to encourage the formation of rock hard breasts. Seeing calcium deposition on the implant capsule (which is what makes the breasts really hard) in a woman who has never had silicone gel implants is extremely rare.

Fixing Implanted Breasts in the OC

Redo Breast Implant Patient by Dr D - Orange County California

Breast implant surgery does not always go as planned. The reasons vary from patients seeking poor surgical talent at their local $3000 chop shop, smoking too many cigarettes or just plain having bad luck.
Much of this can be fixed or at least improved with additional surgery.
This young lady found me after having had four operations with her prior surgeon at which point he uttered those unforgettable words: “This is as good as it is going to get.”

She had poor scarring partially due to her genetics, but the poor shape and firmness was correctable. She found her way to my practice after a friend with whom I had worked in the past referred her to me. We performed a modified breast lift and implant exchange with capsule removal in a local Orange County surgical center and she achieved a nice improvement.

Fixing Bad Plastic Surgery:

** Get into the best hands you can afford and check into your surgeon’s qualifications before you let him operate upon you. Understand that redos are harder and less predictable than first time surgery.

Good Plastic Surgery in the OC – Nice Breast Implant Exchange Revision

Some Orange County women come to consultation interested in breast augmentation with anatomic or tear drop shaped breast implants to look more “natural.” A case in which I was asked to fix the breasts of a woman who had had five “bad boobjobs” (the patient’s words) by another surgeon actually involved removal of her tear drop shaped implants and replacement with round implants in addition to a modified breast lift. The “swapping out” of one breast implant (or pair) for another is called a breast implant exchange:

Breast implants should not touch in the middle

A forum I visited featured this image in a thread of bad boob jobs. This time I agree.

Symmastia
is an unusual problem with breast implants that tends to happen in unskilled hands. I saw a case with a resident I was asked to supervise. In doing her original cosmetic implant surgery he had obviously separated her pectoral muscles from her chest bone (sternum.) The implants in a properly done case would have muscle separating them creating cleavage.

Symmastia can be fixed, but is best done by decreasing the implant size and sewing the scar capsule to itself internally. This diminishes the problem unless large implants are placed again in which case it just tears. Time must be allowed to pass after the first operation for this scar tissue to form providing a platform for the stitches.

Interestingly when using smaller implants and taking one’s time, this problem is virtually never created. :)

Reader Question – Can Breast Implants Fix My Nipples?

As you can see from my pictures my left areola is larger than my right one. Also the left breast is also a bit lower and more “stretched” than the right after my pregnancy and breastfeeding. In my particular case do you think that getting implants would stretch the right areola (which never really changed during breastfeeding) out a little to get closer to the circumference of the left areola? It seems like the left might stay kind of the same since it already has been stretched during breastfeeding the way an implant would have. It would be nice if the nipples were a little closer in size but I would like to avoid any scars around the areola.

Breast implant placement alone does not allow for changes of one breast and areola alone.

It is very common that breasts change during breastfeeding and these changes may not be even from side to side. One of my patients called one of her breasts her “milker” indicating that it was the one her child liked for breastfeeding and used with a preference. This breast was droopier than her other one.

In your case, your left breast is larger and a bit more droopy. The nipple on the side is a bit larger as is the areola. The placement of implants alone may stretch both sides, but there is no way to know by how much. To get your breasts closer in dimensions you would need to have a breast lift as well.

Breast lifts come in different varieties. The more involved (more incisions) leave more scars, but can make more changes to make the breasts more similar. The question of how much correction a woman will desire and her tolerance for potential scarring is a variable one. Some full breast lifts heal very well such as this recent case I operated.

You do not “need” a breast lift but implants alone will not address the differences in your breasts to any significant extent. The placement of implants alone might even make these differences more pronounced.

Tuberous Breasts – How To Fix Them

Tuberous breasts are breasts with a “tubelike” shape. They are also known as tubular or constricted.

The degree to which the shape develops varies quite a bit. Sometimes the breasts are different sizes with very different shapes. One can be very small almost appearing like a little hanging nipple with very little actual breast tissue. As the severity of the problem increases, there is progressively less breast tissue and skin.

The lady on the next page has a mild degree of the problem.

Reader Question: Fixing a Botched Labiaplasty?

I had a labiaplasty a few years ago and I think I am a victim of a botched job. The doctor removed both of the lips completely. I wanted a reduction not a chop job. I feel too embarrassed to let any of my partners look at my vagina. Can it be fixed?

Maybe.

It amazes me how trusting some patients are when they seek a surgeon for labiaplasty. The actions of a novice can really wreak havoc on your privates. Unfortunately less is more in good labiaplasty surgery as the removal of too much is the hardest problem to correct.

There might be a procedure to improve things so I recommend you see a good surgeon local to you who is experienced in labiaplasty just for an opinion. I am happy to see you if you are fairly close. I am in Southern California.

Look at images of your prospective surgeon’s work. I have a dozen or so just of patients who have allowed me to share their results. (I always ask a patient before I show her images to anyone as we are talking about the most private of areas.) Repair labiaplasty is not common, but it is possible in many cases.
