Reader Question: Cougar Plastic Surgery in the OC?

You seem an approachable plastic surgeon. Do you see any of these cougars looking for too much plastic surgery to hunt down their cubs? Lip collagen? Too big breast implants? How can you do it when they look so deformed?
Cougars do not usually come to the office with a declaration of their evening activities. Once in a while an older gal will want to look much younger. Very rarely she will be open about what she intends to do with her new look however.
It is kinda like going to the body shop to get something fixed and being asked about where or how you plan to drive the car afterward. It is really none of my business.
In Orange County it is not like there is a pack of easily identified cougars roaming the offices of plastic surgeons. The question of what I might do with a woman who wanted more than I thought was a good idea of anything is the same.
If what she wants has relatively few bad effects I just do it. If she seems bent on a course that will make her look awful in the long run I decline. Usually I split the difference.
I don’t do Trout Pouts and I’d like the breast implants I place to not have problems if they can be avoided. We do what we can to make it happen that way.

Pressure Wound Surgery in Orange County

When I tell people I have a hybrid plastic surgery practice, I am not talking about a fuel efficient car. I am talking about the fact that I do both cosmetic surgery and medically-necessary surgery. Part of the medically-necessary surgery includes the repair of pressure wounds (also called decubitus ulcers.)
Not only is this type of surgery not cosmetic, it can be pretty ugly. It is surgery to assist often long standing wounds in healing in often significantly ill patients. If possible such a wound is repaired using tissue from adjoining areas of the body called fasciocutaneous and musculocutaneous flaps after scar and debris is cut away. This isn’t pretty, but the patient was pretty happy to not have an open wound afterward.
I do these at an LTAC in Santa Ana at which good wound care nursing services are available before and after surgery. Not all patients are candidates for surgical correction. Often these patients have had these wounds for months or years at the time they are referred for possible surgery.

Do you know a good plastic surgeon for me in the OC?
I wonder if I am spending too much time blogging when I get asked a question like this one….but seriously do you expect me to recommend someone else in my practice’s backyard?
I practice in San Clemente and Orange.
In all seriousness, the answer to your question really depends upon what exactly you are looking to have done. I specialize on breast and body work although I also do “skin” work and some facial cosmetic surgery as well. No one “specializes” in everything. You need to do your homework to find the best for your area of choice and budget of course.

Good Plastic Surgery in the OC – Refreshed Eyelid Surgery

Blepharoplasty is upper and/or lower eyelid cosmetic surgery usually performed for redundancy of the skin and fat of eyelids. Insurance companies may pay for upper lid surgery if the lids get in the way of the patient’s vision. The rate they pay for the surgeon is usually pretty poor.
This young woman came for cosmetic blepharoplasty of the upper and lower lids. This was performed through traditional trans-cutaneous incisions allowing the removal of skin and fat. At 6 weeks she was pleased.

Dr D and an OC Eyelid Skin Cancer Case

Left Lower Eyelid Skin Cancer
My blog visitors occasionally ask if as a plastic surgeon I treat skin cancer. In this economy, I am actually doing a fair amount of skin cancer. Sun exposure in Orange County California is part of our way of life. We could use a bit more sunscreen.
The patient you see to the right here had a skin cancer appear on her eyelid. She wanted a plastic surgeon to remove it and was referred to me by a local doctor. We were able to remove it under local anesthesia (numbing shots) alone.

Good Plastic Surgery in the OC – Liposuction Booty

I take a lot of heat from other plastic surgeons for my “overly negative” posts, so here is a good liposuction result. There are good liposuction results. The vast majority of mine are good or better. Then again I am qualified and take the time to do a good job. You do not see lumps, bumps or dents here.
This young lady came in bothered by her “saddlebags.” She was happy 2 months later. There are more Before & After Liposuction images at my practice web site for those interested.
