Porn Stars and Their Boob Jobs

I wrote an article for “Exotic Dancer” magazine a few years ago in which I discussed the problem with many adult entertainer / porn star breast augmentation outcomes. The funny thing (when looked from outside the practice) is that women in general come to plastic surgeons wanting pretty large breasts. At times, the requests are destined to look unnatural or perhaps even freakish.

Personally I turn away the freakish ones. I don’t do this very often. :) When women “go really large” and have little of their own tissues available to cover the implants they not only end up looking unnatural, but also have a higher chance of hardening (capsular contracture), rippling (showing signs of the underlying implant – visible through the skin) as well as other complications. These complications usually amount to more surgery.

This is all relative; relative to the amount of tissue you have over your chest to begin with (skin, breast, and muscle) compared to the dimensions of the implant you’d like placed. Many women can get the size they want and not end up with problems. If you are very thin though, maybe those DD’s should be off the menu.

Really pretty women can end up with funky-looking breast implants. These kinds of outcomes can be avoided many times by simply choosing smaller implants (or avoiding high profile implants.) Choosing qualified surgical talent and following the guidelines such surgeons offer can be helpful as well.

When patients follow my advice, their implants tend to last ten years or more without additional surgery. Ask how many of your friends have seen that kind of longevity with their implants. I call this low maintenance breast augmentation.

“Porn Star Breast Implants I Want’em” – A Reader Query

Reader Question:

I like your No BS plastic surgery viewpoint, but I want porn star breast implants. I know you can do it. You have done it before.

I can probably do that, but I need to know what exactly you look like and what you want to look like. In essence I need to evaluate you.

This site is meant to educate potential plastic surgery patients so I disclose information that seems negative to some. Being big on disclosure can discourage some potential patients.

Understand that “going larger” on breast implants increases your risk over the long term. You will have a higher re-operation rate over time. At times to get you where you want to go, several operations over time might be needed. The prior patient you linked in your question only required one though. :)

Being honest or “No BS” as you state, means that I feel the need to tell my patients about the “up and down” of an operation before I do it. If the operation seems doomed to fail from the start I tend to decline.

As long as your case seems to be “do-able” and your risk reasonable, I would be happy to take care of you. I am not the cheapest, but my patients believe I am worth it. Thanks for your interest.

Understanding the “Porn Star Boob Job”

We have discussed “all implant” boob jobs before. This is my designation for a woman with implants so large they dwarf her own tissues. Breast implants help make breasts look the most natural when there is adequate tissue to cover them. When the size of the implant gets too large and/or they are put over the muscle, we get distortion and/or a more fake look. The breasts end up looking like balls or oranges. Some women of course desire this look. Many of them work in adult entertainment.

With the “bigger implant and little or no covering tissue” jobs, we also get the increased possibility for re-operation for pain, implant displacement, and rippling. In the breast implant case, “bigger” comes with “bigger risks.”

Women who choose to go progressively “bigger” and “bigger” flirt with disaster at some point. There are always compromises though. The woman in the image above seems far from that, but does have the balloon look – the implants are either over the muscle or have little tissue covering them.

Aylar Lie – Before and After Breast Implant Removal

Aylar Lie has had an unusually interesting life. She has been Miss Norway, a porn star in the US, and more recently a Norwegian celebrity of sorts.

She is interesting from a plastic surgery point of view as she admits to having had breast implants and then later having them removed. There are images with her before and after the removal here.

She appears to have had “porn star variety” high profile implants that are really fake-looking, but then again it was probably at the time she was playing porn star.

For people who have asked the question how a woman may look like after removing her implants, here is an example.

Breast Implants Online – How to Choose.


This gal likely has high profile implants. She probably wanted to look like a porn star.

Type “Breast Implants” into your search engine of choice and you’ll find yourself hurled into a menagerie of porn sites, special interest group sites and plastic surgery ads. It is more dense than it was more than twelve years ago when I put up my first plastics site, Plastic Surgery Interactive. Nevertheless, it seems the misconceptions about breast implants continue. Cosmetic breast surgery can be performed to limit risk and make lasting good results.

Still patients with “30 Year Old Bags” are probably best advised to at least have them changed for newer models.

For the woman entertaining cosmetic breast implants however, it can be done with a long term goal in mind. There are a two main items to consider:

(1) How large do you want your breasts to be? If you want to be quite large, plan on having more surgery over the course of your lifetime. There is a size point at which your tissue simply cannot contain the implants well. At this point you start looking more and more “fake.” Interesting in this discussion is that some women want that appearance in the first place. That is OK, but consider that your choice puts you into “I’ll likely need more surgery often” category.

(2) Implant type/shape/style – silicone gel versus saline. High versus moderate profile versus anatomic. If you want to look like a porn star, then you generally go as large as you can and choose a high profile implant. Silicone gel versus saline is a matter of opinion. I generally still advise saline unless the patient is going quite large.

If you look at the statistics in the implant literature, you see that a 1/3rd of patients are reported to have undergone additional breast surgery over a three year period. This is substantially reduced by limiting implant size and choosing a qualified surgeon. These are your choices. In my practice the re-operation rate is less than 5% over more than ten years. In small saline-filled implant patients it is even lower.

Make sure you are doing this for yourself. Men don’t know the difference between real and nicely done implanted breasts anyway. Look at this poll if you care what men tend to think about the issue. They vary.
