Clean Smile Line Filler – Orange County California

Here is a smile line filler case before and 2 weeks later from my Orange County practice. The anatomic areas treated were the Nasolabial folds and Marionette lines. Not all smile line filler results need to be scary. We used a hyaluronic acid based product.
The biggest problems with filler cases tend to revolve around the large amounts of filler that some practitioner place. Moderation leads to more natural results here. You might not get all your linbes corrected but you will look human when you leave. We look at this as a reasonable trade off.

Here is a modest lip filler case before and five days later from my Orange County practice. Not every patient wants “Trout Pout meets domestic violence” on her lips. In this case we used Prevelle.
The exact filler choice is not as important (as long as a quality cosmetic material is used) in avoiding the trout pout as the quantity and technique. I recommend hyaluronic acid types in the lips to avoid unsightly lumps and bumps. Good technique limits the bruising.

We have discussed here on multiple occasions the confusion in the facial filler market. For quite some time my opinion has been that the hyaluronic acid-based fillers are the safest. There are several on the market.
Well now there is a new one, Belotero Balance from the makers of Radiesse. The study they quote in their materials was against a Bovine based filler which not surprisingly showed that the new product was superior. That is why the Bovine products have fallen by the wayside over the last decade.
Our e-mail to the company has not been answered regarding the issues of cross-linking, lidocaine content or physician sampling.

Jessica Simpson admits to "Trout Pout"

Jessica Simpson came clean on her use of Restylane for her lips. She complained that it looked fake and then that it didn’t last. She admitted to nothing else on other cosmetic surgery though.
Jess, you can always have less injected if you want “natural?”

Lindsay Lohan seems to have developed the “trout pout” once again. I always wonder why these celebrities seem to think this looks good.

The real question is what looks good. It sounds like a silly question but whereas some people want to look natural others want to look “supernatural” in the area of lip fillers just like breast implants. I posted a conservative lip filler “Before and After” on the blog a few years ago and many readers could not tell the difference. They were likely used to seeing much larger Trout Pout lips.
I give patients a mirror when I do lip injections so they can tell me how much they want. Some patients end up with a partially saved syringe for later and for others we use it all.
As usual, it is all relative.
BTW – One of the Daily Mail readers likely had it right. Collagen is rarely used these days. It is far more likely Lilo had one of the hyaluronic acid-based products injected.

Dermal Fillers – The Truth

Questions regarding early interventions to ward off aging are common here at the blog. One of the most common considerations is that of dermal fillers. The market in fillers has exploded over the last 10-15 years. They are being marketing heavily and looked at with more interest in this down economy.
The concept of fillers is simple: plump up tissue that seems too small or saggy. This works to an extent although we have all seen the results when maybe too much has been used…the dreaded trout pout. In reality many early lip injections (even well done ones) look kinda large until they settle down that is unless they are planned in stages to be more discreet.
There are several categories of fillers. The safest also lasts the least amount of time, the hyaluronic acid fillers. These include Hylaform, Captique, Restylane, Perlane, Prevelle and Juvederm. They can be used in most areas and when used properly have a low chance of forming lumps or bumps. Perlane, Prevelle and Juvederm are “cross linked” to last a little longer, but the longevity here is in the 4-6 month range.
Some newer semi-permanent fillers have been introduced that can last up to a year. Radiesse is one such filler with some limitations. It is more likely to form lumps and bumps and is better injected in sessions with small amounts added each time. It is best used in thicker tissues such as the chin and the laugh lines to decrease the chance of lumpiness.
The permanent fillers are not safe. Stay the hell away from them! Silicone injections can be a nightmare in many cases as we have described it before. Checks posts here tagged “Injection Hell” to read more about this if you wish.
The bottom line on fillers is take it easy on the amount and go to a reputable injector to make the experience as safe as it can be.

Disfigured By Fillers

[T]he British Association of Aesthetic Plastic Surgeons (BAAPS) published statistics revealing that almost a quarter of their members had seen patients for surgery to correct problems caused by permanent fillers – injectable gels used to add volume to the face and, more recently, the body.
Permanent fillers are dangerous. Silicone gel was the prototypical “permanent filler.” No responsible plastic surgeons of whom I know offer it to their clients.
The good and the bad about the safe soft tissue fillers is that they don’t last too long. The newer Hyaluronic acid fillers (Hylaform, Restylane, Captique, Prevelle and the like) are probably the safest we have seen to date. They have the fewest reactions and problems, but last only a few months.
Stay away from permanent fillers.

Lip Fillers – Hylaform – A Modest Enlargement

The most common Lip Fillers (Restylane, Hylaform, Captique) are all composed of hyaluronic acid, a naturally occurring substance in skin. They pretty much give equivalent results.
While we have all seen the freakish outcomes in celebrities, here is the other side with a modest enlargement. This patient was injected twice two weeks apart with about half a single unit each time to get the results you see a week after the second injection. She had no bruising and was out and about the evening after each injection. There is swelling that resolves, but in modest enlargements, no trout and no fat lip.
