Good Plastic Surgery in the OC – A Good Redo Boob Job

Here we have images of a woman I first saw a few years ago. She had had two breast implant operations before and noted after her second (an enlargement with full lift) that her left breast progressively dropped (over about a year). I recommended re-operation to re-set the lift and downsize her implants. She agreed to the lift but not the implant size decrease. Smaller implants do not weigh as much so the rate of recurrent sag should have been less. As these were silicone gel implants this is particularly important. They weigh more than saline-filled implants.

The After image was taken at 1 month. She moved Out of State, but I predict that the sag probably recurred at least to some extent.

The Lesson:

(1) Large implants are hard to keep up. Plan on smaller implants or more surgery later if you select larger implants. Large silicone implants also harden (capsular contracture) more frequently/severely than smaller implants or saline implants. This also contributes to a higher redo rate.

Reader Question: Redo Tummy Tuck Pictures?

I am looking into a Redo Tummy Tuck here in the OC. Why do you not have redo Tummy Tuck images on your site?

Revision plastic surgery is an uncommon part of a practice’s cases (in the right practice of course,) but a more difficult part of plastic surgery. And tummy tuck surgery is one of the larger scale operations your plastic surgeon can perform.

Because of these issues the results in redo tummy tuck surgery will vary quite a bit. Most surgeons are a bit hesitant to place these kinds of results on their web sites for fear of scaring away the “first time” tummy tuck patients. Even a good revision case result image tends to pale in comparison to an average first time operation in which a complication did not develop.

Suffice it to say that a good deal in the land of plastic surgery relates to impressions.

Fixing Implanted Breasts in the OC

Redo Breast Implant Patient by Dr D - Orange County California

Breast implant surgery does not always go as planned. The reasons vary from patients seeking poor surgical talent at their local $3000 chop shop, smoking too many cigarettes or just plain having bad luck.
Much of this can be fixed or at least improved with additional surgery.
This young lady found me after having had four operations with her prior surgeon at which point he uttered those unforgettable words: “This is as good as it is going to get.”

She had poor scarring partially due to her genetics, but the poor shape and firmness was correctable. She found her way to my practice after a friend with whom I had worked in the past referred her to me. We performed a modified breast lift and implant exchange with capsule removal in a local Orange County surgical center and she achieved a nice improvement.

Fixing Bad Plastic Surgery:

** Get into the best hands you can afford and check into your surgeon’s qualifications before you let him operate upon you. Understand that redos are harder and less predictable than first time surgery.

Boob Job Redo Video

Boob job redo surgery is certainly more difficult than first time cases. Looking over some of my older YouTube videos, there was a desire to fix some up a bit. I had this boob job redo video with a patient testimonial. This video is now new and improved with reduction of annoying music and re-arrangement of the elements. It tells the story better now.

The story in short was this very nice young woman had been operated by a local surgeon who reported himself as the “best of the best.” Sound familiar? He nevertheless ran into a complication and after three operations left her pretty deformed. He reported to her that “this was as good as it was going to get.” Then she found Dr D through a friend. This was not an easy case and her result is just as much due to my work as her persistence, faith and patience.

Good Plastic Surgery in the OC – Nice Breast Implant Exchange Revision

Some Orange County women come to consultation interested in breast augmentation with anatomic or tear drop shaped breast implants to look more “natural.” A case in which I was asked to fix the breasts of a woman who had had five “bad boobjobs” (the patient’s words) by another surgeon actually involved removal of her tear drop shaped implants and replacement with round implants in addition to a modified breast lift. The “swapping out” of one breast implant (or pair) for another is called a breast implant exchange:

Breast Implant Warranty and Limiting Revision Cost

It is interesting in this economy how some patients are thinking about breast implants. The more limited funds available in a down economy might be good for one thing: people are thinking implant surgery through much better on average than in years gone by.

A forum at which I occasionally answer questions reflected this in a question. The poster indicated that the decision of whether or not to place breast implants with her planned breast lift may revolve around the risk of revisions.

I can’t afford to have revisions. If I have to replace implants I should not go for it then.

As we have discussed previously plastic surgery can be performed with the main goal of limiting the need for further surgery. In the case of breast implant surgery, limiting long term risk can be best accomplished by:

(1) Limiting implant size.

(2) Sticking with saline-filled implants which tend to simply rupture as opposed to encouraging scarring and significant capsule formation that requires more involved surgery to treat. Saline implants break and need simple replacement far more often than anything else at least when they are not huge.

(3) The other issue with saline implants is that rupture can be covered to an extent by buying an extended warranty at the time of their original implantation. This specifically covers breast implant rupture. The main problem that occurs with the smaller saline implants is rupture.

It is true that the extended warranty only covers the implants for ten years, but that is more coverage than you can get with silicone breast implants for the problem that is the most common with saline.

Tummy Tuck Revision – Is It Worth it?

In considering tummy tuck surgery patients reading online can become concerned about the potential desire for secondary surgery. Forums are filled with patients complaining about their bad tummy tuck or botched tummy tuck. I have reviewed some of these and some have legitimate points. Others have chosen less qualified surgeons and their results are pretty average. Some of these lackluster results can be improved.

Revision tummy tuck can be as simple as minor scar revision in the office under local or much more involved surgery under general anesthesia. In Orange County we have a few lesser-qualified surgeons whose patients adorn the schedules of other surgeons looking for tummy tuck revision. There are other docs who just don’t relate well to their clients. I try to stay out of the politics and legal wrangling that can be involved. Patients just looking at the possibilities of improvement are welcome of course.

Katie Price’s Nipple Slip and Reducing Breast Lift Scars

Jordan had just had surgery to reduce her implant size and lift her breasts. Close up images of a nipple and areola after breast lift surgery take time to improve. It is called “healing.”

What do you all expect? Weeks after implant exchange and lift surgery of course things will look funky. This is one of the reasons to think twice before having monster-sized breast implants. They stretch out your skin and if you decide to later remove or downsize them, a breast lift will be needed to “take in all the slack.” Everyone needs to chill.

As an aside, I like the use of fewer sutures over the skin in breast lift surgery to reduce the cross-hatching you see in Jordan’s scar here. As time passes that will get lighter, but will not go away entirely. One of my full breast lift/implant patients is linked below so you can see what I mean. ;)
