I wrote an article for “Exotic Dancer” magazine a few years ago in which I discussed the problem with many adult entertainer / porn star breast augmentation outcomes. The funny thing (when looked from outside the practice) is that women in general come to plastic surgeons wanting pretty large breasts. At times, the requests are destined to look unnatural or perhaps even freakish.
Personally I turn away the freakish ones. I don’t do this very often.
When women “go really large” and have little of their own tissues available to cover the implants they not only end up looking unnatural, but also have a higher chance of hardening (capsular contracture), rippling (showing signs of the underlying implant – visible through the skin) as well as other complications. These complications usually amount to more surgery.
This is all relative; relative to the amount of tissue you have over your chest to begin with (skin, breast, and muscle) compared to the dimensions of the implant you’d like placed. Many women can get the size they want and not end up with problems. If you are very thin though, maybe those DD’s should be off the menu.
Really pretty women can end up with funky-looking breast implants. These kinds of outcomes can be avoided many times by simply choosing smaller implants (or avoiding high profile implants.) Choosing qualified surgical talent and following the guidelines such surgeons offer can be helpful as well.
When patients follow my advice, their implants tend to last ten years or more without additional surgery. Ask how many of your friends have seen that kind of longevity with their implants. I call this low maintenance breast augmentation.

The paparazzi must be busy because we are seeing more rippling these days. We just commented on Victoria Silvstedt’s Rippling Images.
Rippling is just the appearance of a breast implant under thin soft tissue cover. It tends to occur in women with relatively little soft tissue coverage who have had larger implants placed relative to that amount of tissue coverage. It happens more with saline implants and “Over the muscle” placement, but can occur with silicone gel “Unders” if the patient is thin enough and/or the implants are large. Keeping the implant size smaller and going under the muscle tends to limit it.
The good news is that Ms Dickinson is really not rippling too badly. In the more obvious rippling cases, the ripples appear over more of the breast(s) with relatively little movement. I only see her ripples well in one of the images near the top of her implants. It is all relative.

Donatella Versace breast implant rippling is easy to see. She is quite thin and takes in too much sun exposure, so she displays breast implant rippling:
As we have discussed before, rippling is the visible dimpling through the skin seen when breast implants are covered by soft tissue that is on the thin side. It is seen here as Donatella sits upright topless at the beach as a number of roughly vertical parallel straight lines atop her left breast. Rippling is not defined by that which is felt but rather by that which is seen.
Surgery to repair this involves a few maneuvers which are variably successful. It is best to try to avoid it in the first operation by using muscular coverage and limiting implant size. When patients are quite thin it can be harder to hide. It bothers some people more than others.

Tamara Mellon Shows Breast Implant Rippling

Tamara Mellon is by reports quite wealthy, so the fact that she tolerates rippling is a bit surprising.
Rippling is the visible dimpling through the skin seen when breast implants have too little soft tissue coverage. It is seen here as Tamara leans forward as the dimples (four of them here) along the side of her chest at the base of the breast implant on this side.
Surgery to repair this involves a few maneuvers which are variably successful. It is best to try to avoid it in the first operation by using muscular coverage and limiting implant size.

What is Breast Implant Rippling? – Victoria Silvstedt Shows Us

Victoria Silvstedt displays rippling along her left breast cleavage in this image. Rippling is more common in women who have small breasts and go with larger implants. It refers to the appearance of “ripples” seen through the skin in women with implants. It is not the ability to feel the implants, but the sight of these ripples and appears more often with movement.
Rippling happens more commonly with saline-filled implants, but can also show itself with silicone gel implants. It has to do with the tissue coverage being relatively thin compared to the size of the breast implants underneath.
Surgery to minimize this involves a few maneuvers which are variably successful. It is best to try to avoid it in the first operation by using muscular coverage and limiting implant size.
Interesting is the fact that some women do not care whether or not rippling affects them and some (few) men actually like it. As plastic surgeons in training, we are taught techniques to avoid it as it is classically considered a complication.

I found this gal’s image on theperfectboobs.net, a forum dedicated to really large “fake looking” breast implants. By report, she had 1000 cc implants and was looking for larger. She is slender and the rest of her seems in great shape. Obviously she works out quite a bit.
The question is whether or not this size is ever desirable and/or worth the risk. As we have discussed here, more gals with larger implants will have problems and end up looking poorly over time relative to patients with smaller implants. Interesting in the perfectboobs forum is the desire by some to obtain the “fake” appearance that many other women seek to avoid. Some of the posters at that forum even like rippling, a sign resident plastic surgeons are taught to avoid by limiting implant size.
